We’re looking for a passionate and motivated More Than My Past Practitioner to join Forward Trust’s Connections Team at HMP Millsike.
This is a full-time position working 35 hours per week, offering a unique opportunity to support people in custody to improve their physical, mental and emotional wellbeing, build resilience, and develop healthier lifestyles that can be sustained on release.
Working as part of our innovative Health & Wellbeing service, you’ll collaborate with prison staff, healthcare professionals, peer mentors and external partners to create a positive, recovery-focused environment across the prison.
What You’ll Be Doing
- Delivering 1:1 support, group interventions and wellbeing workshops
- Coaching and mentoring service users to achieve positive, lasting change
- Supporting and developing prisoner Health & Wellbeing Champions
- Promoting healthy lifestyles, recovery and wellbeing across the prison
- Organising health promotion events and awareness campaigns
- Building strong partnerships with healthcare, mental health and substance misuse services
- Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
- Maintaining accurate records, care plans and case management documentation

We’re seeking someone who is:
- Passionate about health, wellbeing and recovery
- Skilled at building positive relationships and motivating others
- Confident delivering both one-to-one and group-based interventions
- Organised, proactive and able to manage a varied caseload
- Comfortable working within a multidisciplinary environment
- Committed to safeguarding, inclusion and person-centred support

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
Please note if you have lived overseas within the last 5 years then checks may take longer.
