Are you a compassionate and skilled nurse ready to make a real difference for people struggling with substance dependency?
The Forward Trust is seeking a Band 6 Community Substance Misuse Nurse to join our team in Thurrock. This is a part-time role, 17.5 hours per week, delivering high-quality, person-centred care to people experiencing substance dependency.
You will work as part of a multidisciplinary team, providing and supervising evidence-based substance misuse interventions. Your role will include triage and assessment, brief interventions, prescribed drug treatment, case management, crisis support, group sessions, and 1:1 work. You will also support access to primary care and mental health services, promoting healthier lifestyles and overall wellbeing.
What You’ll Do:
- Provide interventions and support for drug and alcohol users with complex needs.
- Develop, implement, and evaluate recovery-focused care plans.
- Conduct risk assessments, safeguard adults and children, and maintain accurate clinical records.
- Facilitate group and individual sessions using motivational interviewing and behaviour-change techniques.
- Collaborate with other professionals and agencies to ensure integrated care and maintain high service standards.
- Participate in ECGs, fibro scans, clinical audits, and health assessment clinics.
Who We’re Looking For:
- Registered nurse with active NMC registration and substance misuse or community nursing experience.
- Strong communication skills and the ability to manage caseloads effectively.
- Commitment to evidence-based care, professional standards, and continuous development.
